We are a paperless practice. Before your visit, please be sure to establish an account on the patient portal.
Your appointment, whether virtual or in person, will run more smoothly if you complete all the requested information and forms including:
Patient health history, HIPAA and Privacy notice forms, opting in to either email or text notifications
Have any records faxed to us at 833-707-2404
***Telehealth visits are scheduled and billed just as in-person visits. Please expect to check in 5-10 minutes prior to your scheduled appointment time, and complete the necessary paperwork via the portal.
For the safety of the patient, telehealth visits are to be completed in a private area and cannot be completed while driving. These visits are subject to the same cancellation/no-show penalties as in-person visits.
The no-call/no-show fee is $15
